2008 SFFOA Registration Form

NOTE: Only on-line registration will be accepted this year via the website. Registration via paper will be offered at SFFOA meeting prior to the start of the season.

Per SFFOA By-Laws, Membership is by application, certification of registration with the FHSAA, or establishment of good moral character and payment of prescribed fees. This form must be submitted with appropriate fee or it will not be processed.

Registration Fees:

Early Registration (May 28th through July 14th) - $20.00 - CLOSED

Regular Registration (July 15th through September 1st) - $25.00 - CLOSED

Late Registration (after September 1st) - $50.00* - OPEN

* Assignment will be withheld until paid.
